Records

Rates

Dr. Trantham is currently impaneled with Blue Cross/Blue Shield of Massachusetts (HMO, PPO and Indemnity plans) and United Behavioral Health.

• Psychological & Neuropsychological Assessment
  
  Rates for psychological and neuropsychological evaluations will vary depending on the assessment questions and complexities; Dr. Trantham will provide an estimate for the cost of the assessment prior to beginning the evaluation process. Payment in full is due at the feedback session and the psychological or neuropsychological report will be released following full payment for the evaluation.
  
  
• Psychotherapy
  
  Rates for psychotherapy services are $125.00 per hour. If using insurance to pay for psychotherapy services, individual co-pay is due at each session; if paying out of pocket for psychotherapy services, payment is due at each session.

A sliding scale fee agreement is negotiable and based on demonstrable need.

Cancellations and Missed Appointments

Time set aside for therapy sessions or evaluations are not easily filled when they are cancelled without notice. You will be expected to pay for appointments that are not cancelled with at least 24 hours notice; therapy clients will be expected to pay the agreed upon hourly rate, while psychological and neuropsychological testing clients will be expected to pay for one hour at my hourly rate for psychotherapy.

Emergency Coverage

Clients may call my office number 24 hours a day. In the event of a mental health emergency, clients should contact the nearest hospital emergency room. In the event of my absence from the office due to illness or vacation, coverage will be provided and clients will be informed of the covering psychologist's contact information at that time.
